The Follow Up Consultation Discount (“offer”) is valid for bookings made before 11:59pm AEDT 9 December 2024 (“term”), for eligible patient consultations only. Patients who are eligible for this offer will be made aware via email communication at the beginning of the campaign.

1. Limitations

Only one valid offer can be used per transaction.
Offer cannot be combined with any other offers, promotions or discounts.
Offer has no cash value and is non-transferable.

2. How to Use

To be eligible for the offer, you must book your follow up consultation at patient.alternaleaf.com.au before the end of the term.
Offer will be applied to the follow-up consultation fee automatically and will be reflected on the subsequent invoice.
Bookings cancelled and re-booked following the term will not be eligible for the offer.

Firstly, do you have a medical condition that has lasted for longer than 3 months?

Have you previously attempted to treat this condition?

Now a little more about your treatment. Did previous treatment(s) fail to provide full relief or cause unwanted side effects?

And to check, are you currently pregnant, breastfeeding or trying to conceive?

Do you have a history of psychosis, or have you ever been diagnosed with schizophrenia or bipolar type 1 or 2?
